Treatment of the Giant Cell Tumor using Electric Cauterization with Cement

Abstract
Giant cell tumor of bone is a distinct clinicopathologic entity and its histologic and radiologic features are now well understood. These tumors are locally invasive and tend to have a high rate of recurrence, and malignant transformation is well known. The purpose of this paper is to analyse the efficacy of our method with wide opening, through curettage, electric cauterization and bone cement filling in giant cell tumor treatment. 38 cases of giant cell tumor of bone were analyzed which underwent intralesional procedure between 1982 and 1997. Mean follow-up period was 4 years and 5 months. There were 12 distal femoral lesions, 19 proximal tibia, 6 distal radius, and 1 distal ulna. All cases were treated with intralesional procedure: 15 cases had curettage and bone graft, and 5 had curettage and bone cement filling, and ; 18 cases were treated with curettage, electric cauterization and bone cement filling. There were two cases of local recurrence after curettage and bone graft, The recurrence rate after curettage and bone graft or bone cement filling was 15%. ; The curettage, electric cauterization and bone cement filling group hac no recurrence at final follow-up. In this series, there were no pulmonary metastasis but one case was multifocal at presentation. Intralesional procedure of meticulous curettage with wide opening and repeated electric cauterization and bone cement filling seems to be simple, effective and definitive method in obtaining local control of lesions over simple curettage and bone graft or bone cement filling.
